Review Carefully Before Booking This!

You must have proof of Oregon Residency.

 

OMMP only accepts the following documents as proof:

​

  • Government Photo ID:

    • Example: OR Driver's license, Vet/VA ID

    • ​

  • If you DON'T HAVE Oregon driver's license then you must show proof of any of the following:

    • OR Utility bill

    • OR vehicle title

    • OR vehicle registration 

    • Tax statements

    • Mortgage statements

    • Rental lease agreements

(must be signed by both tenant and landlord)

Fees you will pay to obtain license:

​

  • $180 doctor's licensing appointment

  • $200 OMMP Application fee (state of Oregon fee)

    • $380 Total fees required for licensing

​

  • OPTIONAL FEES Paperwork & application assistance fees:

​

  • $35 Paperwork Service Fee (PWSF)

Let our staff do the  work of verifications, preparation and submission of a clean application to OMMP for no-hassle licensing

 

  • $50 Expedited Paperwork & Licensing Fee  

We combine the PWSF service with urgent OMMOS electronic submission to OMMP to obtain your 30Day Temporary License within 24-48rs of your appointment that can be used immediately for dispensaries, DUI & Probation cases, and Employment drug testing policies (*check with your probation & employers first)

If you are NEW to our clinic - we need to collect from PRIOR records from your that will help our doctor understand your conditions - to qualify you for the OMMP license:
