2 Required Fees to Know:
#1 Physician Appointment Fee:

Standard Patient: $180 payable at booking

Discounted Fee: $130 payable at booking (proof of benefits required*)
* Discounted fee requires proof of current Oregon Health Plan (OHP), Food Stamps (SNAP), Supplemental Security Income, or Military Veterans status
#2 OMMP State Registration Fees:
Paid to the OMMP state program for your licensing and registration
Select the lowest fee that applies to you:

Standard patient: $200

Proof of SNAP benefit: $60

Proof of OHP benefit: $50

Proof of Supplemental Security Income (SSI): $20

Proof of Military Veteran Status: $20

Military Vets with great than 50% disability: NO COST FOR APPLICATION

For new patients seeking an OMMP license, we need to gather your medical records for your files for your appointment with our doctor:

If you need us to request records from your health provider, use the button below to complete our Medical Records Release form:
