We created this appointment to specifically help legally document your medical condition that serves are the supporting documents needed for a NEW OMMP application, which is required the OMMP.

Use this appointment if you need a new permanent and legal set of records to document conditions that will qualify you under the OMMP rules.

Qualifying Medical Conditions:
