OMMP APPOINTMENT $180
For NEW or RENEWAL OMMP appointments
You can quickly and easily complete your OMMP paperwork electronically and have your doctor's appointment online to obtain the required documents to submit to OMMP.
DISCOUNTED OMMP APPOINTMENT $130
For NEW or RENEWAL OMMP appointments
Requires proof of Oregon Health Plan, SNAP/EBT, Military Veterans, or Supplemental Security Income (SSI) [not Social Security & Disability]
Medical Records Evaluation & Documentation $50
This visit is for patients who don't have any prior medical records and needing their health conditions documented as required by OMMP. This visit is done PRIOR to an OMMP licensing appointment.
